Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes, following an established care plan. You will help with daily living activities and personal care, ensuring comfort and safety.

This role offers a part-time schedule with flexible hours and a competitive pay rate. You will work closely with patients, families, and supervising nurses to monitor conditions and support independence.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable
• Ability to read, follow written instructions and document care given
• Current CPR certification in accordance with state regulation
• PPD or chest x-ray if applicable
• Successful completion of Maxim screening examinations
• Participation and completion of Maxim’s competency program
• Must meet all federal, state, and local requirements

Responsibilities

• Assists patients with daily living and personal care per plan of care
• Taking and recording temperatures, pulse, respiration and blood pressure when ordered
• Safe transfer and ambulation per plan of care
• Exercises according to the plan of care
• ROM and other simple procedures as ordered
• Patient laundry
• Meeting safety needs and using equipment safely
• Observation and teaching of home safety practices
• Recognition of emergencies and emergency management procedures
• Follow patients’ plan of care and complete tasks assigned
• Communicates clearly with client, family/caregiver and office
• Observes and reports changes to the assigned nurse supervisor
