Job summary

The Lakes Home Care, Inc. is seeking a compassionate Home Health Aide to assist patients in their homes with daily living activities and personal care according to a care plan.

You will support baths, grooming, mobility, meal assistance, and observation while documenting changes and communicating with the supervising nurse.

This part-time role offers flexible hours and the chance to contribute to high-quality in-home care.

Qualifications

• Meets all training requirements in accordance with state and federal laws when applicable.
• Ability to read, follow written instructions and document care given.
• Must maintain current CPR certification in accordance with state regulation.
• PPD or chest x-ray if applicable.
• Successful completion of Maxim screening examinations.
• Participation and completion of Maxim’s competency program.

Responsibilities

• Baths, oral hygiene, dressing and undressing, toileting activities, feeding, etc.
• Taking and recording temperatures, pulse, respiration and blood pressure when ordered.
• Safe transfer and ambulation techniques.
• Exercise according to the plan of care.
• Perform ROM and other simple procedures as ordered.
• Patient’s laundry.
• Meeting safety needs of patients and using equipment safely.
• Observation and teaching of home safety practices.
• Recognition of an emergency situation and knowledge of emergency management procedures.
• Must follow patients’ plan of care and complete all tasks assigned.
• Communicates clearly with the client, family/caregiver and office.
• Observes and documents patient condition and reports changes to the assigned nurse supervisor.
