Job summary

Foundation Partners Group is seeking a Transfer Care Specialist to serve as the first point of contact for families during transfers of decedents from nursing homes, hospitals, residences, or coroner's offices to our funeral homes.

You will coordinate safe transport, maintain professionalism, lift 100lbs safely, and complete required paperwork in compliance with laws. This role requires reliability, empathy, and strong MS Office proficiency.

Qualifications

• Must be at least 21 years old and have a valid state-issued driver’s license with a clean record.
• Ability to lift 100lbs safely as part of daily duties.
• Proficient with Microsoft Office (Word, Excel, PowerPoint).
• Remain composed and empathetic with families in challenging situations.
• Ability to follow detailed written and verbal instructions and apply feedback.
• Ability to establish and maintain effective internal and external work processes.

Responsibilities

• Transport decedents from nursing homes, hospitals, residences, or coroner's offices to the funeral home.
• Provide compassionate, professional service and direct families to resources as needed.
• Navigate local roads safely; use GPS or direction apps; manage time and routes.
• Be on-call for irregular hours, including nights and weekends, and fill shifts as needed.
• Complete paperwork accurately and timely per laws and regulations.
• Maintain a neat appearance and perform related duties such as washing vehicles.
