Job summary

WHITERIVERALF is seeking a caregiver to provide routine daily care and services to assigned residents in accordance with their assessment and care plan. The role involves using PCC to verify resident information during activities such as ALDS and meals, and reporting changes in condition to the Health and Wellness Director.

Qualifications

• High school diploma or equivalent required.
• Proficient in Microsoft Office Suite.
• Ability to read, analyze, and interpret general business materials; ability to write reports.

Responsibilities

• Provide routine daily care and services to residents per care plan.
• Use PCC to ensure correct resident information during care activities.
• Report changes in resident condition to Health and Wellness Director promptly.
• Record entries accurately on notes, SPAs, and incident reports.
• Maintain confidentiality of resident information and report any disclosures.
