Job summary

You will assist with daily living activities, mobility, meals, and personal care, ensuring residents are comfortable, clean, and well-cared for throughout your shift.

Qualifications

• Provide routine daily care and services as per resident assessment and care plan.
• Report changes in residents' condition promptly to Health and Wellness Director.
• Maintain accurate notes, SPAs, and incident reports.

Responsibilities

• Greet residents and escort to rooms; ensure rooms ready (bed made).
• Assist with daily living activities and personal care.
• Observe and report skin integrity, safety hazards, and changes in condition.
• Maintain confidentiality of resident information and report HIPAA violations.
• Assist with feeding, mobility, transfers, and transportation as needed.
• Record and communicate nursing reports and follow policies and procedures.
