Job summary

Sonoma Post Acute is seeking an Admissions Assistant to help provide a smooth transition for new residents and their families. You will support the Admissions Coordinator and work with interdisciplinary teams to facilitate efficient admissions processes.

You will handle inquiries, schedule admissions, verify information, and assist with documentation while maintaining compliance and confidentiality in a busy, compassionate environment.

Responsibilities

• Serve as first contact for prospective residents, families, and referral sources.
• Coordinate and schedule admissions, including pre-admission assessments and docs.
• Maintain accurate records per federal/state regulations.
• Assist with insurance verifications and financial eligibility processes.
• Provide facility tours and information to prospective residents and families.
• Collaborate with nursing, social services, and admin staff to ensure a seamless admission.
• Prepare and distribute admission packets and required documentation.
• Follow up with hospitals and case managers to ensure timely transfer of information.
• Support marketing and outreach efforts as needed.
