You will deliver case management, coordinate services, and develop person-centered plans to help individuals live more independently. Qualifications

• Bachelor's or master's degree in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
• Valid Driver's License and an insured vehicle

Responsibilities

• Provide case management and supports planning services for individuals with disabilities or medical needs.
• Establish and monitor person-centered plans and POS tools.
• Coordinate resources, referrals, and ongoing support services.
• Meet state mandated requirements for the Supports Planner role.
• Maintain HIPAA and confidentiality; document interactions and progress.
