Job summary

Service Coordination, Inc. is seeking Supports Planners in Maryland to provide case management and POS planning for people with disabilities and medical needs.

This role emphasizes person-centered planning, advocacy, and community integration. The position offers a base salary between $64,395 and $79,861, travel reimbursement, quarterly bonus potential, generous benefits including tuition reimbursement, health plans, disability and life insurance, 403(b) match, and substantial paid leave.

Qualifications

• Bachelor's or master's degree in one of: Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
• Valid Driver's License and an insured vehicle.

Responsibilities

• Advocates on behalf of people served and educates them to become more self-determined.
• Collects information and documentation for eligibility for Supports Planning services.
• Plans for and updates Person Centered Plans of Service (POS) with input from people served.
• Researches, refers, and coordinates resources to meet POS outcomes and shares with team.
• Monitors POS and conducts follow-up visits to document progress and update services.
