Qualifications

• Requires a Bachelor's or Master's degree in a human services field (psychology, sociology, social work, nursing, counseling, or human services administration).
• Valid driver's license and insured vehicle are required.
• Position requires case management responsibilities and travel as part of community-based supports.

Responsibilities

• Advocate for people with disabilities and coordinate supports.
• Establish and monitor Person Centered Plans of Service (POS).
• Identify and secure ongoing support services and coordinate resources.
• Engage with the community, families, and providers to meet positive outcomes.
• Complete required paperwork and maintain HIPAA confidentiality.
• Adhere to SCI policies and procedures and maintain compliance with regulations.
