Job summary

Service Coordination, Inc. is seeking dedicated Supports Planners to serve the Bethesda community.

You will provide case management and supports planning to individuals with disabilities, ensuring dignity and rights while coordinating resources and services. Bachelor's or Master’s in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration is required, plus a valid driver's license and insured vehicle.

Qualifications

• Bachelor's or master's degree in Psychology, Sociology, Social Work, Nursing, Counseling, or Human Services Administration.
• Valid Driver's License and an insured vehicle.
• Experience working with people with disabilities preferred.
• Experience with person-centered planning preferred.

Responsibilities

• Advocates on behalf of the people served, provides resource information, and educates them to become more self-determined.
• Collects information and documentation related to eligibility for Supports Planning services.
• Plans for, facilitates, develops, and updates Person Centered Plans of Service (POS).
• Monitors POS and conducts follow-up to ensure ongoing support services.
• Coordinates resources and services to achieve POS outcomes and assists with waivers.
• Ensures HIPAA compliance and maintains records.
