Job summary

Creative Living Options, Inc. in Sacramento seeks a Management-level Community Support Facilitator to coordinate services for individuals with developmental disabilities, ensure IPP/ISP adherence, and promote inclusion in the community.

You will supervise Direct Support Professionals, maintain records, and spend substantial time in the field with consumers to deliver training and oversight, while ensuring emergency preparedness and collaboration with care teams.

Qualifications

• High school diploma or equivalent required.
• One year of Direct Support experience required.
• Valid CA driver license and reliable vehicle.

Responsibilities

• Coordinate services and IPP/ISP endpoints.
• Supervise Direct Support Professionals and ensure duties per job description.
• Maintain detailed records, case notes, and progress reports.
• Spend at least three days weekly in the field with consumers providing support and oversight.
• Prepare and monitor POS authorizations and payroll-related documents.
• Communicate with Regional Center, families, and stakeholders; participate in planning meetings.
