Responsibilities

  • Supervise Direct Support Professionals and train in support, documentation, and safety procedures.
  • Prepare for emergencies, participate in disaster planning, and ensure safety and IIPP compliance.
  • Monitor POS authorizations, attendance, and payroll documents for direct reports.
  • Support ongoing professional development and stay updated on best practices in the field.
  • Maintain regular communication with Regional Center, parents/conservators, and stakeholders.
  • Maintain on-call status and provide on-site support when needed.

As a Community Support Facilitator at Creative Living Options, you will play a pivotal role in ensuring the well-being and satisfaction of individuals with developmental disabilities. You'll be responsible for coordinating services, supporting individuals in their daily lives, and promoting their inclusion within the community.

Philosophy and Values
  • Understand and articulate Creative Living Options' Mission, Vision, and Values.
  • Stay informed about relevant laws and regulations, such as the Lanterman Act, Title 17, and Supported Living Regulations.
Caseload Coordination
  • Ensure individuals receive assistance in all aspects of daily living and the implementation of their Individual Program Plans (IPP), Individual Support Plans (ISP), and health plans.
  • Ensure individuals are satisfied with their services and that all provisions agreed upon in the IPP are delivered. Identify barriers to consumer satisfaction and address them.
  • Prioritize consumers' self-determination, choices, and inclusion in the community.
  • Support consumers in directing and supervising support staff.
  • Maintain detailed records, files, and case notes.
  • Identify and facilitate access to community and generic resources.
  • Complete thorough assessments and debriefings when challenging behaviors emerge.
  • Spend at least three days weekly in the field with consumers, providing support, training, and oversight.
  • Develop meaningful relationships with consumers and their support networks.
Supervision and Coordination
  • Manage and supervise Direct Support Professionals (DSP), ensuring they fulfill their duties as specified in their job descriptions.
  • Train DSP in all aspects of direct support, teamwork, documentation, reporting, behavior support plans, and home maintenance procedures specific to each home.
  • Proactively utilize constructive feedback and corrective action when supervising DSP.
  • Foster positive team working relationships.
Emergency Preparedness
  • Ensure consumers are prepared for emergencies, including disaster planning and safety measures.
  • Provide immediate on-site response in emergencies.
  • Adhere to the Injury & Illness Prevention Program (IIPP).
  • Submit Special Incident Reports (SIR) promptly.
  • Conduct house inspections and ensure homes are safe and well-maintained.
Budget and Payroll Management
  • Prepare, submit, and monitor Purchase of Service (POS) authorizations.
  • Monitor and update personal attendants' schedules and services within the approved POS.
  • Review and approve timesheets and payroll documents of direct reports.
  • Ensure In-Home Supportive Services (IHSS) is maximized and effectively managed for consumers served.
  • Maintain appropriate staff coverage levels, ensuring appropriate backup support is available.
Professional Development and Training
  • Participate in ongoing professional development, including conferences and workshops, to stay updated on best practices in the field.
Communication
  • Submit annual and semi-annual ISP reports and monthly progress reports to the Regional Center.
  • Maintain regular communication with Regional Center representatives, parents/conservators, and other stakeholders.
  • Coordinate and participate in IPP and Person-Centered Planning meetings.
On-Call
  • Maintain on-call status during designated hours.
  • Maintain a mobile phone and respond to calls within 15 minutes.
  • Work in place of a DSP, if required.
  • Support, promote, and participate in CLO-sponsored events and activities.
Qualifications
Mandatory Qualifications
  • High school diploma or equivalent.
  • One year of Direct Support experience.
  • Reliable vehicle.
  • CA driver license.
  • Current California auto insurance.
  • Clean driving record.
  • Active CPR/ First Aid certification.
  • Able to lift/move up to 50 pounds.
Preferred Qualifications
  • Bachelor's degree in psychology, social work, or equivalent.
  • One year experience coordinating services for people with developmental disabilities.
  • One year experience in a Supervisory position.
