Job summary

ReDiscover’s CoEd program seeks a Community Support Specialist to assist adults with mental health or substance use needs in outpatient or residential settings. You will manage a diverse caseload, support symptom management, employment, and independent living goals, and help clients access needed resources.

Work includes coordinating with a multi-disciplinary team, addressing barriers to care, and supporting transportation and documentation within the EMR.

Qualifications

• Bachelor's degree or equivalent experience required.
• Excellent customer service with a recovery focus.
• Strong written and verbal communication skills.
• Proficient with MS Word, Excel, and email.

Responsibilities

• Maintain and interact with a caseload of clients in-person or by phone.
• Facilitate outcomes in symptom management, employment, and independent living.
• Engage difficult-to-serve clients by researching resources and making connections.
• Help remove barriers and connect clients to programs and services.
• Assist clients with transportation barriers, including driving when needed.
• Document interactions in the organization’s electronic medical record.
• Collaborate with a multi-disciplinary team to coordinate client care.
