Job summary

ReDiscover is seeking a Community Support Specialist to work with adults and youth, providing case management and crisis support within a mission-driven non-profit. You will collaborate with a multidisciplinary team to connect clients to long-term supports, while helping them navigate transportation and access to community programs.

Qualified candidates have a 4-year degree or equivalent experience, strong customer service, and excellent communication skills.

Qualifications

• 4 year degree or 4 years of equivalent experience required.
• At least two years of clinical experience in a mental health setting preferred.
• Knowledge of Kansas City metro area resources and health/social service systems preferred.
• Excellent customer service with a recovery focus.
• Strong written, oral, and interpersonal communication skills.

Responsibilities

• Maintain and interact with a caseload of clients, in-person or over the phone.
• Facilitate long-range outcomes: symptom management, employment, independent living.
• Engage hard-to-serve clients by investigating resources to gain access.
• Help remove barriers and refer clients to educational or work programs.
• Assist with transportation barriers and transport clients as needed.
• Collect and document client information in the electronic medical record.
• Collaborate with an internal multidisciplinary team to coordinate care.
