Job summary

Qualifications

• Experience in direct service provision to children and adolescents preferred.

Responsibilities

• Maintain a caseload of clients with diverse needs in-person or by phone.
• Facilitate long-range outcomes like symptom management, employment and independent living.
• Engage difficult-to-serve clients by exploring resources to gain access and connections.
• Help clients participate in community or social service programs and referrals to education or work programs.
• Assist clients with transportation barriers as identified in treatment plans.
• Collect and maintain client information and document interactions in the EMR.
