Job summary

ReDiscover’s Transitions Program is seeking a Community Support Specialist to support individuals in recovery from substance use disorders in Kansas City, MO. You will engage clients in person and by phone, helping them access housing, employment, and education while reinforcing MAT goals.

You’ll document progress in Carelogic and collaborate with a multidisciplinary team to promote stability and positive outcomes in a compassionate, mission-driven setting.

Qualifications

• Bachelor’s degree in a related field, or four years of equivalent experience in lieu of a degree.
• Two or more years of experience in a substance use, behavioral health, or related setting preferred.
• Knowledge of community resources and health/social service systems in the Kansas City metro area preferred.
• Strong ability to build relationships and engage clients in a recovery-oriented environment.
• Excellent written, verbal, and interpersonal communication skills.

Responsibilities

• Manage and actively engage a caseload of clients receiving treatment for substance use disorders through in-person and phone interactions.
• Support clients in progressing through recovery by reinforcing treatment goals, relapse prevention strategies, and overall stability.
• Build trust with hard-to-engage clients and promote consistent participation in treatment services, including medication-assisted treatment (MAT).
• Help reduce barriers to recovery by connecting clients to housing, employment, education, and other community supports.
• Coordinate care and provide transportation to treatment appointments, including MAT services, using your personal vehicle as outlined in client treatment plans.
• Document client progress, services, and interactions in the Carelogic electronic health record (EHR) system.
• Collaborate with a multidisciplinary team, including medical and clinical staff, to support continuity of care and positive treatment outcomes.
