Job summary

Humana Inc in La Crosse, WI is seeking a Community Resource Coordinator to provide comprehensive case management for members in the Wisconsin Family Care program. You will assess health and safety needs and develop a Member Care Plan with the care team.

You will conduct scheduled in-person visits, coordinate services for social integration, housing, employment and other supports, and maintain accurate documentation in electronic case-note systems.

Qualifications

• Bachelor's degree in human services or related field is required with at least 1 year of experience serving frail elders or adults with disabilities.
• Alternative: Bachelor's in another field with at least 3 years of relevant experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan.
• Coordinate with a Field Care Nurse to provide services in the least restrictive environment.
• Conduct face-to-face social assessments upon enrollment and at least every six months; quarterly in-person visits.
• Maintain monthly member contact by phone and document updates to MCP and care notes.
