Job summary

Humana Inc. is seeking compassionate, organized case managers to support Wisconsin Family Care members. You will assess health and safety needs, develop MCPs, coordinate with a Field Care Nurse, and conduct regular in-home and phone follow-ups.

The role requires a Bachelor's in human services or related field (or equivalent experience), plus a valid driver’s license and insurance. Travel up to 40% is expected; typical hours are Mon–Fri 8:00–4:30 CST.

Qualifications

• Bachelor's degree in human services or related field required with at least 1 year serving frail elders or adults with IDD; or Bachelor's in another field with 3+ years such experience.
• Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs.
• Conduct face-to-face social assessments at enrollment and at least every six months (typically at member home).
• Conduct quarterly in-person visits and monthly phone contact.
• Arrange non-medical services, including social integration and housing.
• Ensure cost-effective service delivery.
• Evaluate risk factors and provide education to members.
• Maintain MCP documentation and related records.
