You will work in the member’s home or partner facilities, coordinating care plans and social supports with a RN supervisor, including travel within Jefferson County, WI. Qualifications

• Bachelor's degree in Human Services or related field with 1 year of relevant experience, or a degree in another field with 3 years of relevant experience.
• Experience serving Family Care target populations: frail elders or adults with IDD or physical disabilities.

Responsibilities

• Learn what is important to the member and support them in achieving it.
• Partner with RN Health and Wellness Coordinator and coordinate member plans.
• Conduct face-to-face social assessments at enrollment and at least every 6 months.
• Coordinate quarterly visits and monthly phone contacts.
• Develop and review the member's plan of care.
• Lead on social, community integration, employment, housing, and non-health issues.
• Educate on prevention and mitigate risk; document activity and paperwork.
