Job summary

Humana Inc. seeks a Community Resource Coordinator to support Wisconsin Family Care members with comprehensive case management for frail elders and adults with disabilities. You will develop and update the Member Care Plan and coordinate services with the Field Care Nurse.

This remote role requires travel up to 40%, typical hours Monday–Friday 8:00 am–4:30 pm CST, and a valid driver's license with insurance. Competitive pay and benefits offered by Humana.

Qualifications

• Must have at least 1 year serving frail elders or adults with disabilities; acceptable with 3+ years in another field.
• Valid state driver’s license and car insurance with 100/300/100 limits.

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with Field Care Nurse to deliver services per MCP in least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months.
• Conduct quarterly visits and maintain monthly phone contact with members.
• Arrange non-medical support services such as housing, employment, and social integration.
• Maintain accurate documentation including MCP updates and service authorizations.
