Job summary

Humana, in collaboration with Inclusa, is seeking a Case Manager to serve Wisconsin Family Care members in Richland County and nearby areas. You will work with a Field Care Nurse to implement a comprehensive Member Care Plan and support care in the least restrictive environment.

The full-time role requires a Bachelor's degree in human services or a related field (or equivalent experience), a valid driver's license with current insurance, and a track record with frail elders or adults with

Qualifications

Bachelor's degree in human services or related field required.
1 year of experience serving frail elders or adults with disabilities, or 3 years in related field.
Valid state driver's license and proof of personal vehicle liability insurance (100/300/100).

Responsibilities

Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
Coordinate with a Field Care Nurse to address health needs in least restrictive environment.
Conduct face-to-face social assessments at enrollment and at least every six months.
Perform quarterly in-person visits and monthly phone contacts with members.
Arrange social, housing, employment and other non-medical supports.
Ensure cost-effective service delivery and evaluate risk factors.
Maintain accurate documentation including case notes and MCP updates.
