Job summary

Humana is seeking a RN Care Manager to support Wisconsin Family Care members. The role involves care coordination, health assessments, and education, with no night or weekend shifts and a Monday–Friday schedule in La Crosse, WI.

Responsibilities include developing care plans, coordinating with providers, and educating members on conditions, nutrition, exercise, and healthy lifestyle changes. Travel up to 40% is required.

Qualifications

• Associates degree in nursing required.
• 3 years care management experience preferred.
• Licensed RN in Wisconsin in good standing.

• Ensure newly assigned RNs and their Care Coach partners are informed of follow-up needs.
• Accept additional complex member assignments.

About This Role

As a RN Care Manager, you will support members enrolled in the Wisconsin Family Care Program by providing essential care coordination, support, and education, all while enjoying a true work-life balance with no night or weekend shifts.

Highlights

• Location: La Crosse, WI
• Schedule: Full-time, Monday through Friday, 8:00 am - 4:30 pm CST
• Travel: Up to 40%
• Pay: $71,100 - $97,800 per year
• Sign-On Bonus: Eligible for a bonus incentive plan

What You'll Do

Requirements

  • Licensed Registered Nurse in the state of Wisconsin (in good standing) required
  • Valid state driver's license and proof of personal vehicle liability insurance required
