Job summary

Humana Inclusa is seeking a Case Manager to support Wisconsin Family Care members in Richland and Iowa Counties. You will assess needs, develop a Member Care Plan, and coordinate with a Field Care Nurse to provide services in the least restrictive setting.

You will perform enrollment assessments, conduct regular visits, arrange non-medical supports, and maintain detailed documentation, ensuring cost-effective service delivery and member well-being.

Qualifications

• Bachelor's degree in human services or related field required, with 1 year of relevant experience.
• Alternatively, a Bachelor's in another field with at least 3 years of relevant experience.

Responsibilities

• Assess members' health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address health and safety needs in the least restrictive environment.
• Conduct face-to-face social assessments at enrollment and at least every six months, usually at the member's residence.
• Conduct quarterly in-person visits and monthly phone contact.
• Arrange non-medical supports like social integration, housing, and community resources.
• Maintain accurate documentation including MCP updates and service authorizations.
