Job summary

Humana is seeking a Community Resource Coordinator to support Wisconsin Family Care members in La Crosse. You will perform comprehensive case management, coordinate with Field Care Nurses, and conduct regular home and clinic visits to ensure holistic, person-centered care.

You will document care plans, track progress, and connect members with community resources while maintaining cost-effective service delivery. The role requires travel and continuous collaboration with the Inclusa/Humana team.

Qualifications

• Bachelor's degree in human services or related field required; 1 year of experience with frail elders or adults with disabilities.
• Bachelor's degree in another field acceptable with at least 3 years relevant experience.

Responsibilities

• Assess health and safety needs to develop a comprehensive Member Care Plan (MCP).
• Coordinate with a Field Care Nurse to address members' needs within MCP guidelines.
• Conduct face-to-face social assessments upon enrollment and at least every six months, typically at the member's home.
• Conduct quarterly in-person visits and maintain monthly contact with members by phone.
• Arrange support services for social integration, housing, employment, and other non-medical needs.
• Ensure cost-effective service delivery and budget adherence.
• Evaluate risk factors and provide education to members.
• Maintain accurate documentation including case notes, service authorizations, and MCP updates.
