Job summary

Trilogy is seeking a Peer Support to assess and support clients across home, community, and office settings in Chicago. The role focuses on motivational, strengths-based, trauma-informed care for individuals with severe mental illness and co-occurring disorders using harm-reduction approaches.

The position requires engagement with the client’s support network, coordination of recovery plans, and documentation in Trilogy’s EMR while advocating for access to services and resources in the community.

Qualifications

• Must have a High School Diploma or GED with two years of supervised clinical experience in a mental health setting.
• Must have an Associate’s or Bachelor’s degree in any field.
• Must be a past or present consumer of mental health services and willing to share their personal recovery story.
• Valid Illinois driver’s license with daily access to a well-maintained vehicle and $100,000/$300,000 liability insurance.
• IM+CANS certification preferred and may be required to obtain IM+CANS certification within 30 days of hire.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client, client’s support system, and other care providers.
• Model for clients good communication skills and recovery-oriented living skills.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and MAR documentation in real time.
