Job summary

The Trilogy is seeking a Peer Support in Chicago, IL to assess clients across home, community, and office visits and to provide motivational treatment and advocacy to support recovery from mental illness.

The role emphasizes client-centered, strengths-based, and trauma-informed services, links clients to community resources, and requires timely documentation in the EMR. A $1,500 sign-on bonus is offered.

Qualifications

• Must have one of the following requirements: High School Diploma or GED with two years of supervised clinical experience, or an associate/bachelor’s degree.
• Past or present consumer of mental health services and willing to share recovery story.
• Valid Illinois driver’s license with daily access to a vehicle and adequate liability coverage.
• Experience with mental health and/or substance use disorders preferred; IM+CANS certification preferred.

Responsibilities

• Assess clients’ personal, medical, emotional, social, and environmental situations through home, community, and office visits.
• Coordinate the creation of an individual recovery plan with the client and care providers.
• Model effective communication, recovery-oriented living skills, and coping strategies.
• Participate in Peer Support meetings and activities; provide crisis coverage as needed.
• Complete daily progress notes and documentation in MAR/Electronic Medical Records.
• Assist clients with housing, entitlements, budgeting, and accessing community services.
