Job summary

Trilogy Careers seeks a Peer Support specialist to assess clients and develop recovery plans through home, community, and office visits. You will advocate for clients, provide trauma-informed services, and coordinate with healthcare teams to ensure medication accuracy and appropriate care levels.

The role emphasizes client-centered support, crisis coverage, and documentation duties in line with Trilogy’s policies. A CRSS credential is preferred and a valid IL driver's license is required.

Qualifications

• Must have one of the following: High School Diploma or GED and two years of supervised clinical experience in a mental health setting or an Associate’s/Bachelor’s degree.
• Must be a past or present consumer of mental health services and willing to share recovery experience.
• Valid Illinois driver's license with daily access to a vehicle and $100,000/$300,000 liability insurance.
• Experience in community-based services preferred.
• IM+CANS certification preferred / may be required within 30 days of hire.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and support system.
• Model effective communication, recovery-oriented living skills, and coping strategies.
• Participate in Peer Support meetings and activities.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and maintain timely documentation.
• Document medication administration records (MAR) and coordinate with nursing staff.
• Assist clients in independent living skills and entitlements applications.
• Accompany clients to appointments and advocate for client needs.
