Job summary

Responsibilities include creating recovery plans, documenting in MAR/EMR, providing crisis coverage, and coordinating with nurses. Must have HS diploma or higher; CRSS within 12 months; valid IL driver’s license and daily vehicle access.

Qualifications

• Must have one of the following educational requirements.
• CRSS credential within 12 months of hire.
• Valid Illinois driver’s license with daily access to a vehicle and insurance.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situations through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and care providers.
• Model recovery-oriented skills and effective coping strategies.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and documentation in EMR/MAR.
