Zusammenfassung

Trilogy, Inc. in Chicago IL is seeking a Peer Support to assess clients across home, community, and office visits, guiding recovery for those with severe mental illness and co-occurring substance use disorders.

You will provide trauma-informed, strengths-based services, develop individual recovery plans, complete timely MAR documentation, and collaborate with nurses, case managers, and other providers to connect clients with community resources and entitlements.

Qualifikationen

• Must have HS diploma or GED or higher education.
• Illinois driver’s license with daily access to a vehicle and adequate insurance.
• Experience in mental health and/or substance use settings preferred.

Aufgaben

• Assess client’s personal, medical, emotional, social, and environmental situation across home, community, and office visits.
• Coordinate recovery plans with client and care team.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and documentation (IMCANS, LOCUS, MAR) in real time.
• Educate clients on treatment, medications, and entitlements; assist with housing, budgeting, and services.
