Job summary

Trilogy, Inc. is seeking a Recovery Counselor - Peer to assess clients and plan treatment across home, community, and office visits in Chicago, IL. The role emphasizes trauma-informed, strengths-based, client-centered care and links clients to community services.

You will provide 75% community-based services, maintain caseload-based case management, and offer crisis/on-call coverage while advocating for clients and supporting recovery from mental illness and co-occurring disorders.

Qualifications

• Bachelor's degree in counseling or related human service field or equivalent.
• Bachelors in any field with two years of supervised clinical experience in a mental health setting.
• LPN license under Illinois Nurse Practice Act.
• Certificate of Psychiatric Rehabilitation from DHS-approved program; high school diploma/GED with two years’ mental health experience.
• CRSS certification.
• Family Partnership Professional certificate with IAODAPCA.
• OT Assistant license with one year in mental health setting.
• High school diploma/GED with five years supervised clinical experience in mental health.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation via home, community, and office visits.
• Coordinate establishment of an individual recovery plan with client and care providers.
• Model strong communication, recovery-oriented living skills, and coping strategies for clients.
• Participate in Peer Support meetings and activities to gain support in the role.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and MAR documentation per policy and standards.
• Document care using MAR and electronic medical record systems.
• Assist clients with housing, entitlements, and money management per agency policy.
