Job summary

Trilogy, Inc. is seeking a Peer Support specialist to assess clients and plan individualized recovery paths across home, community, and office settings.

You will advocate for clients, link to community services, and provide strengths-based, trauma-informed support for those with serious mental illness and co-occurring substance use disorders. You will maintain patient records, participate in team meetings, and be on-call for crises, while helping clients access entitlements and develop

Qualifications

• Must have one of the following: high school diploma/GED with two years supervised clinical experience, or an associate/bachelor's degree.
• Must be a past or present consumer of mental health services and willing to share recovery experiences.
• Valid Illinois driver's license with daily access to a vehicle and $100,000/$300,000 liability insurance.
• IM+CANS certification preferred and may be required within 30 days of hire.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client, support system, and other care providers.
• Model recovery-oriented living skills and effective coping strategies.
• Participate in Peer Support meetings and activities to gain additional support.
• Provide crisis/on-call coverage as directed.
• Complete daily progress notes and documentation per agency policy.
• Assist clients with entitlements and navigating community services.
• Transport clients to appointments and support housing, budgeting, and medication needs.

Education

High School Diploma or GED
