Job summary

Trilogy is seeking a Peer Support to assess and support clients with serious mental illness and co-occurring disorders. The role includes coordinating recovery plans, linking to community services, and providing client-centered, trauma-informed care using harm reduction.

Responsibilities include documentation, crisis coverage, and assisting clients with entitlements while collaborating with a multi-disciplinary team in a community-based setting.

Qualifications

• High School Diploma or GED with two years of supervised clinical experience in a mental health setting.
• Associate’s or Bachelor's degree in any field.
• Past or present consumer of mental health services and willing to share recovery story.
• Valid Illinois driver’s license with daily access to a vehicle and $100,000/$300,000 liability insurance.
• Experience with mental health and/or substance abuse diagnoses preferred.
• IM+CANS certification preferred and may be required within 30 days of hire.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client, their support system, and other care providers.
• Model effective communication, recovery-oriented living skills, coping skills, and self-help strategies.
• Participate in Peer Support meetings and activities to gain additional support in the position.
• Provide crisis/on-call coverage as directed and identify signs of burnout or overwhelm.
• Complete daily progress notes and documentation in the EMR (MAR, IMCANS, LOCUS).
• Assist with referrals, entitlements applications, and case management within a shared caseload.

Education
