Job summary

Trilogy in Chicago seeks a Peer Support to assist clients with mental health and co-occurring disorders. You will perform home, community, and office visits, develop recovery plans, provide trauma-informed, strengths-based support, and act as an advocate linking clients to community resources.

Responsibilities include completing daily progress notes, crisis/on-call coverage, coordinating with nurses and pharmacies, and helping with independent living skills.

Qualifications

• Must have one of the following requirements: HS Diploma or GED with 2 years supervised clinical experience or an Associate/Bachelor’s degree.
• Must be a past or present consumer of mental health services and willing to share recovery story.
• Valid Illinois driver's license with daily access to a vehicle and $100,000/$300,000 liability insurance.
• Experience with mental health and/or substance use diagnoses preferred.
• IM+CANS certification preferred; may be required within 30 days of hire.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and support system.
• Model recovery-focused communication and coping skills.
• Participate in Peer Support meetings and activities; obtain additional support.
• Provide crisis/on-call coverage as directed.
