Job summary

Molina Healthcare is seeking a dedicated Community Health Worker to provide non-clinical paraprofessional support in the field, advocate for underserved members, and help navigate health plan benefits. The role involves in-home visits, teamwork with the health services unit, and outreach to social services to improve care access.

Ideal candidates bring at least 1 year in health care, CHW or CMA credentials where applicable, and strong communication, organization, and computer skills.

Qualifications

• At least 1 year of health care experience with underserved populations.
• CHW certification may be required depending on state/contract.
• Valid driver's license, reliable transportation, and adequate auto insurance.
• Ability to multi-task while assisting members.
• Excellent customer service and communication skills.
• Strong organizational and time-management abilities.
• Willingness to learn other programs and software systems.
• Proficiency in Microsoft Office applications.

Responsibilities

• Advocate for members and assist with health care needs.
• Meet with members in homes, facilities, or provider offices.
• Help navigate and maximize health plan benefits.
• Assist with social services access (housing, food, transportation, etc.).
• Outreach to locate/disconnect members needing support.
• Conduct outreach and research to reach hard-to-contact members.
• Travel locally 50-80% as required by state/contractual needs.
• Guide members in maintaining Medicaid eligibility and financial resources.
