Qualifications

• Bachelor's degree in a health care field required.
• Minimum 2 years of clinical case management experience.
• Minimum 1 year of MS Office experience (Word, Excel, Outlook).
• Strong organizational, communication and time-management skills.
• Ability to travel up to 75% within an assigned territory.

Responsibilities

• Coordinate member care needs for medically complex patients.
• Apply evidence-based clinical practices to case management.
• Deliver self-management support and patient education.
• Ensure appropriate utilization of health benefits and services.
• Conduct telephonic and face-to-face assessments per policy.
