Job summary

Molina Healthcare is seeking a Community Health Worker to support community-based member advocacy and act as a local resource for vulnerable members. You will engage with members, help navigate benefits, and collaborate with the care team to improve access to services.

The role emphasizes travel within the community (up to 80%), outreach initiatives, and coordination with social services to ensure comprehensive care and Medicaid eligibility maintenance.

Qualifications

• At least 1 year of health care experience, preferably with underserved or special needs populations.
• CHW certification may be required for certain states.
• Valid and unrestricted driver’s license, reliable transportation, and adequate auto insurance for job travel.
• Ability to multitask while speaking with members.
• Excellent customer service skills.
• Strong organizational and time-management skills.
• Willingness to learn other lines of business, programs and software systems.
• Excellent verbal and written communication skills.

Responsibilities

• Engage with members as an advocate and resource to support health care needs.
• Collaborate with the health care services team providing paraprofessional duties in the field.
• Assist members in navigating and maximizing health plan benefits and arranging services.
• Help members access social services such as housing, food, and employment resources.
• Provide outreach to locate and support disconnected members with special needs.
• Conduct research to locate members using available data and outreach.
• Participate in ongoing or project-based activities requiring outreach.
• Guide members to maintain Medicaid eligibility and other financial resources.
• Travel may be required (50–80% locally according to state/contractual requirements).
