Job summary

Molina Healthcare seeks a Community Health Worker to support community-based member advocacy and navigation of health plan benefits. The role involves in-person outreach, home visits, and collaboration with care teams to improve access and accountability for vulnerable members in Ohio.

The position requires strong communication, organizational skills, and the ability to travel locally (50-80%). A valid driver’s license and health care experience with underserved populations are preferred.

Qualifications

• At least 1 year of health care experience with underserved populations.
• Valid driver's license, reliable transportation, and auto insurance for job travel.
• Excellent customer service and communication skills.
• Ability to multi-task while speaking with members.

Responsibilities

• Engage with members as an advocate and resource to support health care needs.
• Collaborate with the health care services team providing non-clinical paraprofessional duties in the field (home visits, shelters, provider offices).
• Help members navigate and maximize health plan benefits (scheduling appointments, arranging transportation, prescriptions, follow-up).
• Assist members in accessing social services (housing, food, employment, etc.).
• Provide outreach to locate/disconnected members with special needs.
• Conduct research using available data to locate hard-to-contact members across databases and community locations.
• Participate in ongoing or project-based member outreach activities (telephonic and/or face-to-face).
• Guide members to maintain Medicaid eligibility and other financial resources.
