Job summary

Molina Healthcare, Inc. is seeking a Community Health Worker to support community-based member advocacy activities.

You will engage with members in homes, shelters, provider offices, and other community settings to navigate health care needs and maximize benefits. Responsibilities include collaborating with the health care services team, assisting with transportation and appointment scheduling, and connecting members to social services.

Qualifications

• 1 year of health care experience, preferably with underserved or special needs populations.
• Excellent customer service and communication skills.
• Ability to multi-task while speaking with members.
• Valid and unrestricted driver’s license and reliable transportation.

Responsibilities

• Engage with members as an advocate and resource to support health care needs.
• Collaborate with health care services team to provide non-clinical paraprofessional duties in the field.
• Help members navigate and maximize health plan benefits and arrange transportation, appointments, and follow-ups.
• Assist members in accessing social services and community resources.
• Reach out to locate and support disconnected members with special needs.
• Conduct outreach and research to locate hard-to-reach members.
• Travel 50-80% locally as required by contracts.
