Responsibilities

• Secures information and updates consumer treatment plans.
• Provides supportive 1:1 or group counseling.
• Maintains case records and develops measurable service plans.
• Documents progress and follows up on service plans.
• Encourages adherence to medications and appointments.
• Accompanies consumers to medical and government appointments.
• Plans and coordinates activities and referrals.
• Performs crisis assessments and interventions.
• Participates in drills and safety activities.
• Complies with agency policies and attendance.
