Job summary

ICL in New York seeks a Community Support Specialist to assist individuals with mental disabilities living in independent apartments. You will help them access medical, social, educational, financial and vocational services to maximize independent living in the community.

You will travel to residences, provide crisis intervention, document progress, and develop service plans in collaboration with the program team to support discharge planning and ongoing community integration.

Qualifications

• Bachelor's degree or higher with relevant human services experience.
• One year practicum or related direct-service experience preferred.

Responsibilities

• Visits consumers in residences to provide support and counseling.
• Maintains case records, service plans and progress notes.
• Develops and implements individualized service plans within regulatory guidelines.
• Provides crisis intervention and refers to community resources.
• Documents incidents and maintains program compliance.
