Job summary

ICL inc in Brooklyn, NY is seeking a compassionate case management professional to support individuals in independent living. You will travel to residents' homes, develop service plans, coordinate medical, social and vocational resources, and monitor progress to maximize independence.

This role requires strong communication, crisis intervention skills, and the ability to work with diverse populations under supervision.

Qualifications

• Bachelor's degree or higher in social work, psychology, or related field.
• Experience delivering direct services to individuals with disabilities preferred.
• Ability to maintain accurate client records and case plans.
• Familiar with community resources and service coordination.
• Ability to work independently with supervision and adhere to policy.

Responsibilities

• Travel to residents' residences to provide supportive services and counseling.
• Develop and review quarterly service plans and document progress.
• Maintain up-to-date case records and records of treatment plans.
• Provide 1:1 or group counseling to modify attitudes and behaviors.
• Navigate medical, psychosocial, and social needs through community resources.
• Use cognitive-behavioral techniques to assist clients with plans of action.
• Explain clinical services and daily living skills; assist with discharge planning.
• Perform crisis assessment and intervention.
• Refer consumers to resources; accompany to appointments if needed.
• Prioritize safety; report incidents and complete forms per policy.
• Assist in orientation of new personnel.
• Attend meetings and trainings; comply with infection control.
• Advocate for consumers and support vocational training opportunities.
• Other duties as assigned.
