Job summary

ICL inc in Brooklyn, NY seeks a dedicated professional to support consumers living in independent apartments in the community. You will help residents access medical, social, psychosocial, educational, financial and vocational services to maintain independence.

Responsibilities include crisis intervention, developing treatment plans, documenting progress, and coordinating resources with a multidisciplinary team to empower residents toward recovery and community integration.

Qualifications

• Bachelor’s degree plus two years’ experience providing direct services to individuals with mental disabilities or other relevant human service experience, or bachelor’s degree with a required practicum.
• Experience working with mental health or related populations.
• Ability to maintain case records and comply with regulatory guidelines.

Responsibilities

• Travels to consumer residences to provide supportive services per program schedule.
• Develops and reviews consumer treatment plans and quarterly service-plan updates.
• Maintains up-to-date case records and documents progress and outcomes.
• Provides individual or group counseling to support independent living and community integration.
• Assists consumers in accessing medical, social, psychosocial, educational, financial and vocational services.
