Job summary

The Salvation Army North and Central Illinois Division is seeking a Part-Time Case Manager for the Central Illinois area (Canton). The role focuses on outreach, intake, assessment, direct assistance, and case plan development to support clients.

You will document progress in HMIS/SIMS, coordinate with funding sources, and ensure compliance with program requirements. Strong communication and confidentiality are essential.

Qualifications

• Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred.
• 3–5 years knowledge and work experience in Social Services and case management preferred.
• Familiarity with local resources, Continuum of Care and homeless service providers.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or referrals.
• Provide case management services including case plan development and compliance; act as liaison to community resources; arrange referrals and transportation.
• Maintain access to a case manager via appointments and walk-in visits; meet with clients weekly or as needed.
