Job summary

• Experience working with program target population (preferred).

• Facilitates the case management of participants within program guidelines; develops rapport and partnership with participants; develops case plans and supports participants in establishing goals.
• Maintains working relationships with community agencies to promote services; coordinates services and communications; ensures valid releases of information.
• Creates case plans and helps participants achieve identified goals through resources and referrals.
• Enhances personal knowledge through forums and training.
• Uses electronic database to maintain accurate case records and monitor funds.
• Fulfills survey requests and provides information for reports.
