Job summary

Qualifications

• Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred, or significant related experience may be considered in lieu of a degree.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or referrals.
• Provide case management services including assessment of client needs, direct assistance, and case plan development.
• Coordinate with community resources and arrange client transportation as needed.
• Document client information and progress through HMIS or similar systems.
• Maintain client records ethically and confidentially and ensure program compliance.
