Qualifications

• Bachelor’s degree in social work, sociology, criminal justice, psychology or human service preferred.
• A minimum of three to five years knowledge and work experience related to social services and case management preferred.
• Familiarity with local resources and homeless service providers is a plus.

Responsibilities

• Conduct client intake interviews to determine eligibility for programs or referrals.
• Provide case management including intake review, assessment of needs, direct assistance, and case plan development.
• Coordinate with community resources and arrange client transportation as needed.
• Maintain client records and document progress in HMIS/SIMS or equivalent databases.
• Ensure compliance with DHS, HUD, United Way, FEMA, and funder requirements.
