Job summary

NextStep Care in Conyers, GA, seeks a dedicated Case Manager to coordinate home and community-based services for members. You will liaise with providers, educate families, and monitor wellbeing to adjust care plans as needed.

Collaborating with primary care teams, HCBS providers, and families, you’ll document interventions accurately and support ongoing member engagement and functional status. Training new staff is also part of the role.

Qualifications

• Bachelor's degree in social work or related human services field with 2 years of work experience.
• Experience in social work, home and community-based services, healthcare or geriatrics preferred.

Responsibilities

• Liaise between members and home/community-based providers to coordinate care and outcomes.
• Educate members and families on community resources and services.
• Develop collaborative relationships with primary care providers, HCBS, and families.
• Maintain regular communication with members via calls and home visits to monitor wellbeing and adjust care plans.
• Document interventions and member information accurately per guidelines.
• Collaborate with Central Intake and Assessment Team to ensure continuity of care.
• Identify and resolve care plan variances to ensure effectiveness of care plans.
• Address member engagement and behavior to support health and function.
• Participate in after-hours on-call rotation as assigned.
• Support training and onboarding of new team members.
