This full-time role emphasizes access to needed services, development of daily living skills, and crisis response, with duties including assessments, documentation, and family collaboration to support independence in the community.

Qualifications

• BA/BS in a human service field or equivalent experience and education.
• Demonstrated interpersonal skills with adults who have severe mental illness.
• Completion of background checks and pre-employment screening.

Responsibilities

• Identify strategies and options to support clients with mental illness.
• Provide medically necessary case management to restore stability and independence.
• Assist clients in accessing medical, social, educational, and other services.
• Meet engagement expectations and deliver services per level of care.
• Process referrals within 3 business days and document assessments.
• Attend training and provide mobile crisis response as needed.
• Complete additional duties as assigned.
