Many Rivers Whole Health is seeking an Adult Case Manager in Helena, MT. The role emphasizes delivering compassionate, recovery-oriented behavioral health services to adults with serious mental illness and coordinating resources in the community.

You will develop individualized plans, advocate for client needs, and provide intensive home- and community-based care while ensuring thorough documentation and compliance with program requirements.

Competenze

• Bachelor's degree in human services or related field.
• One year of experience serving individuals with mental illness or equivalent.
• Knowledge of case management principles and community resources.
• Strong verbal and written communication skills.
• Ability to develop effective service plans and document accurately.

• Provide case management to adults with serious mental illness.
