Job summary

Bluebonnet Trails Community Services is seeking a Case Manager to provide recovery-oriented services under the supervision of the Clinical Program Manager. You will conduct psychosocial assessments, develop recovery plans, and connect clients with community resources.

The role emphasizes direct service, documentation in the electronic health record, and collaboration with a multidisciplinary team. Required are a Bachelor’s in Human Services (or equivalent with 24 credit hours in a related field)

Qualifications

• Bachelor's degree in Human Services or related field with 24 credit hours if not in Human Services.
• Strong understanding of mental health diagnoses and treatment.
• Ability to work independently and collaboratively in a fast-paced, recovery-oriented environment.
• Valid Texas driver’s license and ability to maintain approved status.

Responsibilities

• Conduct psychosocial assessments and develop Person-Centered Recovery Plans.
• Connect youth, adults and families with community resources and rehabilitative services.
• Initiate crisis intervention when necessary and document in the EHR.
• Provide recovery-oriented services with a multidisciplinary team.
• Prioritize direct service hours (at least 55%) and maintain professional documentation.
